Case 1 - Missing a Premolar Tooth – immediate implant and temporary tooth placement
This patient had her maxillary premolar extracted and the tooth socket grafted in order to allow for implant placement without incisions. Because of the bone graft the ridge is very wide and ideal for implant placement.
After a gentle local anesthetic, a tissue punch was used to remove a small circle of gingival tissue to allow development of the implant site.
The implant is placed with minimal trauma to the tissues.
A temporary crown was placed immediately after implant placement.
After four months a final crown was fabricated.
This xray shows the final implant, abutment, and crown, with excellent bone preservation around the implant.
Case 2 - Missing several teeth on one side of the upper jaw
This person desired a fixed set of teeth to replace 4 missing teeth in the left side of her upper jaw. Four implants will be placed and temporary teeth placed on the implants at same visit as the implant placement surgery.
After the four implants were placed a temporary set of teeth were cemented over the implants.
After four months impressions were taken and four final crowns were made in the laboratory.
The final four crowns look natural and have reconstructed her missing teeth.
Case 3 - Replacement of a fractured left mandibular second molar.
This patient had a fractured left second molar that needed extraction
The tooth was removed. This photo shows the loss of bone that can occur when a tooth has fractured and then becomes infected.
A graft was placed into the extraction site and allowed to heal. After the site heals impressions are taken and a temporary tooth is prepared in the laboratory.
Four months after the tooth was extracted and the site grafted, an implant is placed in the ideal location. The graft at the time of the tooth extraction reconstructed the lost bone allowing for ideal implant location.
Immediately after the implant was placed, a temporary tooth was secured to the implant. This tooth does not touch the opposing teeth, and is present only for esthetics and for patient comfort.
After four months a final crown is made to complete the replacement of the tooth.
Case 4 - Patient with thin ridge and missing teeth in the posterior mandible
This patient is missing three teeth in the right side of the lower jaw. She has a thin ridge which requires grafting to widen the ridge to allow dental implant placement
Under local anesthesia, a tunnel is created and the graft is injected next to the bone to widen the ridge. The procedure takes approximately ten minutes and has minimla pain for the patient.
After four months for bone to form within the graft, the ridge width is now satisfactory for dental implants.
Two implants were placed and a temporary set of teeth secured to the implants.
After the implants have healed the implants are ready for the final bridge.
The final bridge is made in the laboratory
The final bridge is placed and the teeth can now be used to chew a normal diet.
Case 5 - Replacement of two mandibular teeth with ct guidance and immediate provisionalization.
This patient desires replacement of his missing two mandibular teeth, with minimal swelling and pain, and immediate replacement of the teeth.
A ct scan was obtained. Dr. Block created a
3-dimensional virtual bone model and placed two implants.
On the ct generated xray the implant can be placed without going into the nerve which lies within the bone.
A cross section image allows the placement of the implant on the computer in the best position.
After the surgery has been completed on the computer, the plan is sent to the manufacturer and a computer guided stent fabricated to guide implant placement.
Using the computer generated guide stent, the provisional restorations are made in the laboratory, prior to the actual implant surgery on the patient.
The surgical stent is used to guide implant placement. A very minimal incision and flap is made to allow for preservation of vital tissue on the ridge.
The implants are placed in approximately 20 minutes and the provisional set of teeth placed at the same visit. The patient leaves with a fixed set of teeth.
One week later the patient has minimal evidence of the previous surgical procedure.
Case 6 - Immediate replacement of missing front teeth using ct guidance and minimal invasive surgery
This patient presents with missing and decayed teeth. She desires a fixed set of teeth but would like to minimize her pain and swelling after implant placement.
A temporary bridge was made using special material which shows up on the ct scan. The ct scan is then obtained. This xray shows the planned location for the implants.
The implants are positioned on the computer to allow the best esthetic options for the temporary and final set of teeth.
This cross section shows one implant placed in the ideal place and angulation for the final set of teeth.
Using the computer generated surgical guide, implant analogs are accurately positioned into the model. The provisional set of teeth are then made prior to placing the temporary teeth.
This is the computerized surgical guide stent which is used for implant placement.
At surgery the surgical guide stent was used to place the implants without incisions to avoid postoperative swelling.
After the implants are placed the abutments were placed into the implants.
The temporary set of teeth were then placed immediately after the implants were placed.
The patient leaves with an esthetic smile and minimal swelling.
A few days later the temporary teeth look very natural.
